JOB SUMMARY:  Provide continuous and comprehensive services necessary for the promotion of optimal mental health, prevention and treatment of psychiatric disorders and health maintenance to patients of all ages. This includes the assessment, diagnosis, and management of mental health problems and psychiatric disorders. Employee works collaboratively with other on site medical and mental health providers as well as with community PCPs and other provider personnel.

 

Essential duties and responsibilities include but are not limited to the following:

 

  1. Patient Care:
    1. Work collaboratively with a physician and/or mental health provider of record (where applicable) to provide comprehensive mental health services to FMC patients.
    2. Through patient evaluation, determine a diagnosis and plan for intervention.
    3. Adhere to standards set forth in office practice standard protocols.
    4. Maintain a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
    5. Maintain confidentiality and patient privacy.
    6. Document care provided appropriately and meet standards for chart reviews
  2. Consultation and Referral:
    1. Seek consultations as appropriate, act as a consultant has requested.
    2. Facilitate the process of ethical decision making as appropriate.
    3. Perform comprehensive assessments and refer patients for further care and services where applicable.
  3. All Other Duties As Assigned

 

QUALIFICATIONS:  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill and/or ability required.

 

Education:  Masters prepared Nurse Practitioner in Psychiatry with appropriate licensure.

 

Interpersonal Skills:  Strong customer service skills.  Communicates well and supports the organization’s employees, physicians and leadership.  Is able to communicate effectively in English both verbally and in writing.  Requires adaptability with regular interpersonal contact.

 

Decision Making Ability: Work directed by detailed and specific instructions.  Unusual situations are reviewed with a supervisor. Ability to function independently with regards to most clinical decision making.

 

Analytical Ability:  Routing, sorting, filing and appropriate documentation of clinical information.  Concentrates and pays close attention to detail.

 

Required Physical Effort:  Physical and mental stamina to endure the strain and pace of activities.  Must be in good health.  While performing the duties of this job, the incumbent is frequently required to sit; to use hands to finger, handle, or feel objects, tools or controls. The incumbent is frequently required to stand; and walk. The incumbent is occasionally required to climb or balance; and stoop, kneel, crouch or crawl. Exerts 20-50 lbs of force occasionally and/or 10-25 lbs of force frequently.
