If you were enrolled in Franklin Medical Center’s Employer Provided Medical Plan in 2025, you may request a copy of your 2025 Form 1095C from the Human Resources Department.
Requested Form 1095Cs will be distributed by January 31st or within 30 days upon a request being submitted to the Human Resources Department, whichever is later. All requests should be made directly to the Human Resources Department.
